Fronto ethmoidal mucocele with intracranial extension endoscopic decompression

 

65 year old female diabetic came with c/o swelling involving left eye of 10 years duration.  CT scan showed selling of left frontal sinus with erosion of its roof with extension into the anterior cranial fossa.  Left orbital contents were found pushed outwards and downwards.

Surgical decompression of mucocele was performed using a nasal endoscope under general anesthesia. This clipping will show mucoid material extruding on decompression. When the Frontal sinus ostium was widened brain tissue can be seen because of the erosion of the posterior table of the frontal sinus. The brain tissue can be seen pulsating in synchrony with respiration.
